Outcome 3
Number of people linked to care and other support services

Measure
Total number of successful referrals and total number of people referred

Change Process: (self-reported)

1. We seek to reduce the number of new infections and decrease the number of persons at high risk for acquiring or transmitting HIV by increasing people’s knowledge about how HIV, STDs, and TB are transmitted. We also hope to reduce stigma about HIV/TB infection, because stigma is considered to be the main obstacle to prevention.

2. We seek to refer people to HIV-antibody, STD, and TB tests, thereby increasing the proportion of infected people who know they are infected.

3. We seek to cooperate with other providers, thereby increasing the proportion of infected people who are linked to appropriate prevention, care, treatment, and support services.
